Scheduling:
After your initial phone consultation with Speakeasy BKLYN, please complete the intake form. Print and complete the HIPPA and policy form below. Then you may schedule your appointment here.
Insurance & Payment Policy:
Speakeasy BKLYN is an out of network provider. An invoice will be provided at the end of each session which can be submitted to insurance companies for reimbursement. Make sure to contact your insurance company to ask if they reimburse for out of network providers. Payment is due the day of service.
Cancellation Policy:
Speech and language disorders require frequent and consistent work. However there are times when it is necessary to cancel. There is no fee for cancellations made more than 24 hours before your appointment. Cancellations made with less than 24 hour notice will be charged at 100%.
